OK, lets backup here...

Using your other Mac via the SATA to USB adapter can you see the drive using Disk Utilities? Correct? If you can reformat the drive and create a new GUID partition map as well as create a Mac OS Extended (Journaled) partition. Once done from this same Mac, follow these steps: How to install OS X on an external drive connected to your Mac While this TN is assuming you are using it as a spare boot drive the steps are the same for setting up a fresh drive for another system. Jump to the 'Install from your normal startup disk"

I would start off replacing the HD cable as they tend to breakdown. Here is the IFIXIT guide: MacBook Pro 13" Unibody Early 2011 Hard Drive Cable Replacement and this is the cable you want as it's the improved version: MacBook Pro 13" Unibody (Mid 2012) Hard Drive Cable.

But before you do that visit any Apple Store as Apple has been replacing them for free for a lot of people. Put the original drive back in before you go.

Let us know how it goes.

thanks for your reply, i didn't mention that i connected the ssd via usb and tried to boot that way, didn't show up (hold command during startup) i also tried that on my imac (mid 2011, 21.5) it didnt show up either, i formated the drive with disk utility like it said in alot of youtube videos, didnt show up either, but my bootable usb always shows up

Bummer ;-{ Sometimes when the cable goes it messes up the drive too. If you can't reformat it its time for a new drive. As its a new samsung 850 EVO drive you should be able to return it where you bought it to get a new one or contact Samsung for a replacement under warranty.

I'm sure the drive is fine. i have to agree with Dan on this and would change the cable or you could even hook up the SSD to a SATA to usb adapter and try to boot ... if you get the same issue, it is not your cable. usually if its the cable you will get a file with a question mark instead of the apple as the computer cannot see the SSD.

@rebootgozo - FixPal already gave the USB connection a try. Sadly, I suspect the drive has bit the dust.

shame ... :( its quite rare that an SSD failed...
