David, this is something you really should be able to find for yourself rather than asking random strangers to look up the very same information available to you.

A google search took literally 30 seconds to locate the information you appear to be looking for.

Include your name, address for return, phone number, e-mail address, and a brief description of the issue. Laser Health Technologies will contact you with an estimate of the cost to complete any repairs, if the unit is not under warranty, and arrange for payment.

By the way, that's way too much information; we don't need to know all about how you want a checkup, fix the tip cover, check the battery life and check the power wheel. What on earth are any of us, none of whom are associated with LaserHealthTechnologies whatsover, supposed to do with that information? We have no idea how much it's going to cost or how long it's going to take; those are questions to ask once you find the right person to talk to, not us.

Finally, not a good idea to post personal contact information on a public forum; that's an open invitation to identity theft and spam.
