Xbox says controller is disconnected

My Xbox continually says that my controller is disconnecting, even though the light on the controller is still on. I’ve used 3 controller, multiple batteries. I’ve used the USB port in the xbox to charge the controller and it still happens. I’m not exactly sure if this is an Xbox issue or a controller problem. I ordered a new xbox controller and its currently on the way so maybe that will fix it. But any idea will be greatly appreciated. Thank you.

Can you confirm if you are connecting this to an xbox console or PC please?

Also, can you confirm if what you are using to connect the controller, is it always wireless or do you ever use a USB type cable?

Also, are you using battery packs, as in a pre-made after market battery rather that the traditional AA size standard batteries?

It is an Xbox one console. It is a wireless controller but i have used the USB port in the console to charge the controller and still no luck. I mainly use battery packs but even with the issue I’ve been having it happens even when I’m using packs or AA batteries. Thank you!
