I'm still looking into this, but in the meantime it appears Google has an app specifically designed to test the proximity sensor. I ran across the following information on a Google Pixel Phone Help support page here.

Here's the gist of the response.

[quote|format=featured]Make sure the screen is clean. It is possible that something may be blocking the proximity sensor, such as a screen protector; but even a smudge or a fingerprint can interfere with its proper functioning, but there may be other issues as well.

The proximity sensor can be tested by using an app such as the Proximity Sensor Test 2 app, available from the Google Play Store:
If it turns out that the proximity sensor is not functioning as it should, then it's possible it may be a hardware related issue, in which case you would need to contact Google Support.[/quote]

I'd start by running the test then come on back and let us know what it reports.

Also, I did run across another post on a Google site that says the proximity sensor is located just to the right of the front facing camera (#5 on the diagram).
