try cleaning the headphone jack out.. sometimes lint builds up in there and tricks your phone into thinking headphones are in.

Turn the phone to the side and notice a small black tab on the side of the phone on the left hand side near the top. Look in the user manual on page 42 http://manuals.info.apple.com/en/iPhone_.... It's the slider for the manual ring mode selection. See if it's set on silent.

at the bottom of the iphone, they are 2 speaker meshes, maybe they are blocked or covered ? try to clean it with a tooth brush, if not, u need to replace your speaker box. :)
