Can I replace it?

Yes the headphone jack is replaceable. But before you jump there double check if you have the logic board connector clipped in properly. Review Step 54 in the IFIXIT guide here: iMac Intel 21.5" EMC 2805 Logic Board Replacement

Can I replace the jack's cable? Headphones work perfect.
Yes, here's one source: iMac 21.5" Headphone Socket and Cable (Late 13/14)

Can you give us some more information here. Does your internal speakers work without any problems? Can you describe the sound.

The internal speakers work perfect.
The problem is with headphones, the sound is like in an empty room, very very loud in right channel, both channels with large echo. Like la little explosion when connected.

Unless you damaged the jacks cable or the connector I don't see this as the issue. Did you inspect it yet?
Do these headphones work in other devices without a problem? It's possible the line cable is pinched somewhere as you clearly have a signal leakage issue.

Before doing anything see if the headphones do the same on another computer.
